On the fourth leg of the 1,000-mile trek, explorer Benedict Allen and his team face the harshest Siberian winter in living memory. At the most dangerous point, they are lucky to survive after one of the dogs develops a potentially fatal neurosis. Finally, they reach the next settlement, but the strain is starting to tell on Allen's two native guides.

The NHS in Britain is short of 20,000 nurses. In Spain, however, there is a surplus. This Trouble at the Top special follows a group of nurses from Seville as they take up posts in London and Stockport hospitals.
