There have been chaotic scenes in Rio, where the Olympic torch relay was challenged by hundreds of demonstrators angry at the high cost of hosting the Games. A woman has died and five other people were injured in a knife attack in central London. The US Republican presidential candidate, Donald Trump, has insisted his campaign is united, in the face of reports of growing frustration with him amongst the Republican leadership.
