Health correspondent Matthew Hill investigates the tragic errors that left a Wiltshire man brain damaged. Jamie Merrett was so worried about the standard of care he was receiving after a car crash that he arranged for a camera to be set up next to his bed. The footage he captured showed an agency nurse switching off his life support machine by mistake, leaving him gasping for air. 

Plus, on the 50th anniversary of the Severn Railway Bridge Disaster, Professor Mark Horton pieces together the events that led to the loss of five lives and a vital rail link. The accident happened when two ships collided on a foggy night on the River Severn. 

And the last wild amibition of Somerset cancer patient Jack Thomas, who has always wanted to be a rock and roll singer.
