BBC Scotland investigations correspondent, Mark Daly, hears the stories of patients who've suffered for years, or even died, because doctors misdiagnosed their illnesses. He asks why Scotland's NHS doesn't record cases of misdiagnosis, and what can be done to reduce them. He also speaks to doctors who want to see a change in the culture of the NHS and the way it deals with medical errors.

Experts have confirmed to the BBC that the current rate of misdiagnosis for all consultations is around — that's one in six of patients walking through hospital or GP's doors. Annually, that amounts to around 3m consultations in Scotland alone that result in a misdiagnosis. Although not all of those will end in harm to the patient, nobody knows how many actually do, because nobody records misdiagnosis or cases of misdiagnosis, even if they have led to serious harm.
