In this episode, we follow special operations paramedics as they respond to a major incident on the A90 near Dundee. A crash involving a van and a bus has led to multiple casualties, including one patient thrown from the top deck of the bus. Team leader Mark must prioritise who needs the most urgent care as he works with dozens of other emergency responders at the scene of the crash.

In forestry woodland in Lanarkshire, a quad-biking accident tests the ambulance service’s specialist off-road vehicle to its limits as paramedic John struggles to reach the injured patient. Meanwhile, in Edinburgh, paramedic Martyn and technician Kevin are called to treat a woman who has fallen in the street, but without location information they are forced to hunt for her in the busy city streets.

Lauren and Allan in Falkirk are sent to an unusual location when a call comes in for a patient in trouble at the vets, while in Glasgow, John and Suzanne rush to the scene of a nasty bicycle accident.
