In February 1996, 21-year-old Reur Lombard became the first woman firefighter to die on duty in Britain. An inquiry into her death called into question the reliability of her breathing apparatus but, as Chris Choi reports, firefighters are still not equipped to the highest standards because Europe can't agree on new regulations.
